Why have we never seen a real-time, linked to real world prices, shipping game, a la Railroad Tycoon for the 21st Century?

This is no nerd's question. Look at The Economist on April 4th; in "Online Gaming in China: Intangible Value", they describe the first company to list on NASDAQ this flush-year (Changyou) as a gaming business whose "source of profit is receiving cash for items that do not exist". Those profit equaled USD 108 million per year, at last count. There's money in games.
